My HC for this moment—Rin’s continued hesitation to leave Obito—is that she was deliberating on whether or not to mercy-kill him. She was heartbroken to leave her best friend, of course, but there seems to be genuine contemplation in her eyes. She’s a medical ninja. She knows the reality of battlefield injury, hence why she didn’t even attempt to administer any healing. It was over for him. Unlike Kakashi, who wants to escape immediately, she lingers behind, considering his circumstances: he’s blind, bleeding out, scared, about to be buried alive under several tons of rubble. It’s a slow, terrifying death. Mercy-killing appears to be somewhat normal for shinobi; Naruto attempts to kill Haku after Haku loses his will to live, after being unable to convince Haku that his life is worth something. Rin, in a way, chose to be mercy-killed herself. She could have attempted to suppress the 3-Tails—being a Jinchuriki isn’t an immediate death-sentence, so long as you have the correct seals, and her teacher happened to specialise in seals of that kind—but her first instinct was to die as quickly as possible at the hands of a friend. In Obito’s position (facing certain doom) she chose to be taken out swiftly. I wonder if she regretted being unable to offer him the same grace—especially after her death, watching his life descend into misery and being unable to intervene. She was able to gouge out his eye at his request, but I don’t think she had the courage to actually end his life (nobody does: that’s why she had to trick Kakashi into killing her, because she knew it was an impossible ask).
