@crystalthesylbreonart asked if we could get a blank version of the Caine’s paperwork WIPs I mentioned to fill out with OC information, so here y’all go! :D

Plus a version that’s FULLY blank so that you can go ham with it:

Yeah I know this probably doesn’t reflect real hospital paperwork by any means, but hey, we’re having fun!

Also, here are the scrapped WIP’s I was talking about for anyone who doesn’t know what I’m talking about! I might redo them another time, but for now they have really outdated lore that I decided against, so don’t use them as reference for the au lore :,D

Caine’s notes are mostly accurate tho, just missing a lot of additional info that he would’ve actually written down. Y’all aren’t beholden to this or anything, but when I was writing them, my idea was that the notes are color-coded, with red representing symptoms, blue representing things like likes and dislikes that Caine learns about the patient, orange representing action Caine thinks should be taken, and purple representing him speculating about a diagnosis. He takes his notes with one of those multicolor clicky pens.

The freaking fandemonium trio but also not the fandemonium trio.

(Their names are Spigg, Vern, and Moka respectively)

Also lore yapping below. Its alot.

Moka (she/they/it) has always been an outcast on Plant Island. Nobody on plant island really was like her. Nobody was really pink (obviously, there was exceptions, but not like it). Nobody had pompoms. Nobody had any similar fur pattern. Not to say differences are bad, but still it was isolating to be the only one of your supposed species. Also they had quite the personality. Moka was loud, energetic, and just wanted to find somewhere to belong. The Plant residents, to their credit, had tried. But there was something off about its sound. It simply didn't really fit in with their song they had throughout the generations.

Moka was raised by two Bowgarts, unable to have a monsterling of their own. When its egg randomly appeared near the breeding structure, they decided to raise it as their own. Due to this, she does know how to play their instruments. However she doesn't really play much. Typically she's helping them out in their small plant buissness or just trying to get in on whatever is happening on Plant Island.

Despite everything, she still has an incredibly positive outlook on life, wanting to one day find somewhere where she can fit in. For now, she's simply trying to figure out what to do with her life. With her two best friends of course!

Sprigg (they/it) is an extremely nervous furcorn. They always must triple check everything and double decide every choice. They help out at their families seed shop, which is one of the few places they feel calm. They also enjoy wearing their sweater, even if they don't have arms. They enjoy their life on Plant Island, even if its boring. But sometimes it can't help but question what else is out there. Maybe something abit more.... ethereal? But its probably false!

Vern (He/They/it) is pretty much considered the most ordinary Mammott around. He lounges around, talks to others, sings, helps out in baking, and then goes back to bed. Cause thats just life on Plant Island. Nothing really changes. But what if it could? In their freetime, they enjoy reading old legends and myths from the elders and library.

So yeah, those are the main three for the au (for now)! Just three monsters who all have a desire to perhaps see the world, but there probably isn't anything really ever since the Cataclysm. After all, everyone else was wiped out in the Cataclysm!
