I think we should overthink generally how to approach transitional medicine to get the best care for all dysphoric people.
First up, away with affirmation-only. Do not feel threatened by "I need my hormones or I'll kill myself". Compassionate but firm therapists who help explore those feelings and unravel the web of how social dysphoria often functions (by internalised misogyny) and also give alternatives to medical transition.
Second, there needs to be a lot of better research for the effects of cross-sex hormones on bodies, of surgeries and their effects etc pp. Detransitioning people also need more appreciation, more guidance, not just "ok so you've made a mistake"
Generally said, we need more compassion and exploration. We could all benefit from this.
