what is your thoughts on some person with a preference for genitals that has not been surgical altered but it is not relation to gender

It honestly doesn't make sense. How do you know what genitals are surgically altered? And even if you can tell from scars (ie phalloplasty scars on the arm), what makes them any different from natal genitalia?

Like, let's really get into this here.

Is it a natal clitorophallus, or is it a metoidioplasty/phalloplasty without vaginectomy, a penile preserving vaginopasty, a person with bottom growth + a vaginectomy, etc?

Is it diphallia, or is it a phalloplasty + metoidioplasty creating two phalluses, or a person with penile splitting creating diphallia?

Is it someone on the agenital spectrum, or is it a person with nullification?

Is it a person with a natal penis + erectile dysfunction requiring a penile implant for erection, or is it a person with a phalloplasty?

I could go on and on. Literally what is the difference between these? Preferring natal genitalia over surgically altered genitalia comes down to just transsexism and ableism (towards people who have had genital repairs or genital alterations due to genital/sexual dysfunction.)

Heyyy i loved your post abt your surgery, as a transguy that actually the most detailed info ive ever run into & was very informative ty. Im like 5 years into hrt and have been out for 10, surgery is really really scary for me so tysm for demystifying it. Also sorry im on anon im very anxious.

I want to ask a sort of invasive question & if youre uncomfoterable answering plz feel free to ignore me!! I was wondering how much sensation you get on the skin and if its uh pleasurable without "getting hard."

Also is it weird i want to keep my vaginal opening and also have a penis? But im very he/him/masc whatever. Idk i feel self conscious about this 😭 and also worry like, no surgeon would agree to that.

Regarding sensation, I’ve written a few asks addressing this and would suggest that this one below may answer your question:

Retaining your vaginal opening (sometimes you may see references to this as “front hole” if you are searching for further info!) is not an uncommon type of phallo— it would be a phalloplasty with no vaginectomy. I know of a number of folks who’ve opted for this approach and have certainly seen folks talk about this on tumblr so you are not alone there!

The consideration with phalloplasty with no vaginectomy is that some surgeons may be unwilling to offer this if you are also seeking UL due to risk of complications. If you are consulting around this procedure with a surgeon and they seem unfamiliar or unwilling a consult with another surgeon may prove useful! But it’s also good to consider that there may be limitations and individual clinical considerations that impact what surgical options may be safe or accessible to an individual.

I'm still new to thinking about bottom surgery options and I know if I ever did go for it I'd want vaginal preserving metoidioplasty but I can't see any information about whether a scrotoplasty would be possible with that?

A scrotoplasty is possible, although it may be necessary to leave it slightly separated at the rear (i.e. not one continuously joined scrotum) due to the limitations of space in that area. It's unlikely to be noticeable visually when looking straight on.

Vaginectomy: Critical Info for Trans Men

In the context of gender-affirming surgery, Vaginectomy is a Colpectomy plus a Colpocleisis.

Colpectomy involves removal of the vaginal lining, or epithelium;

Colpocleisis is the fusion of the vaginal walls, which creates support for pelvic organs.

The vagina itself isn’t removed though that is done in cancer surgery. In that context, a partial Vaginectomy removes part of the vagina (upper or lower vagina) while a total Vaginecomy removes all of the vagina.

There are several reasons why trans men and non-binary individuals opt to have a Vaginectomy, including:

The desire to have the vagina removed and have a male perineum.

To eliminate the secretions produced by vaginal mucosa.

To eliminate pain from a gynecological condition.

To eliminate the need for speculum exams or Pap tests.

For many years, Vaginectomy endured a reputation for being a very risky procedure—so risky, that many surgeons refused to perform it and even discouraged trans men from seeking it out, citing major intraoperative bleeding and bladder perforation as high risks.

Recent reports from surgeons experienced with the procedure indicate that with modern techniques Vaginectomy may not be as risky as previously thought, and can reduce complications associated with urethral extension. By eliminating vaginal secretions and improving blood flow to the urethra by fusing the vaginal walls, the surgical sites—especially the critical urethral junction—heal better.

A 2018 journal study confirms that Vaginectomy decreases the rate of urethral fistulas.

"A urethral fistula developed in 111 of the 232 patients (48%)  without Vaginectomy and in 13 of the 62 (21%) who underwent primary  Vaginectomy. Secondary Vaginectomy resulted in 100% fistula closure  when performed in 17 patients with recurrent urethral fistula."

Because of this, many--but not all--surgeons now require a Vaginectomy for patients undergoing Metoidioplasty or Phalloplasty IF Urethral Lengthening is being performed.

I am already for sure planning for a hysto, but now am seriously considering metoidioplasty. I'd want a vaginectomy, mons recession or liposuction and testicular implants as well. The thing is, I'm a very large fat man and I want to stay that way. I don't know how that will affect the surgery or if a surgeon will perform on me.

Is it possible if not metoidioplasty, at least a vaginectomy and testicular implants?

But gosh. The idea of being able to finally have a penis. To possibly STP. To no longer having such severe bottom dysphoria every time I think about my genitals. To feel like home. Could it be in my future?
