It was raining.
Which in itself isn't bad. Infact Ratchet usually loves the rain when he's inside, nice and dry that way. However, this had been the day that he promised he'd take Optimus to the park. The kid loved the park, or more specifically loved this little area somehow untouched by humans where he could sit and watch the ducks in a nearby pond. He'd giggle and point, bouncing in excitement. It was unbearablly adorable and Ratchet kept every memory of their time there close to his spark. And after a stressful week, Sentinel being an aft head, humans being a HUGE pain in the aft, seventeen SEPORATE Deception attacks, and he could tell that Optimus was not recharging well or fueling nearly as often as he needed too. In short the kid both deserved and NEEDED his time in his favorite place. But it was raining, and it would do nothing to help him if he was wet and miserable the entire time. OR if Ratchet was either wet or miserable. If only Optimus could see this too. "Bu' bu- pak!" The little fire truck was pointing at the door fluid already in his optics threatening to drip down his faceplate.
Ratchet felt his spark twist in sympathy. "Kid you're gonna get sick if you try and go out in this. It'll get under your plating and feel all icky and cold," he tried to gently reason with the prime, but there was no consoling him. "Shhhh hey don't cry." His hands cupped his faceplate gently and rubbed his digits under his optic. "I know I know. You were really excited weren't you."
Optimus burst into tears as soon as Ratchet said that and nodded before burying himself into his caretaker's neck cabling. Ratchet didn't stop him, in fact he pulled him closer and let the poor prime cry out. He could feel exhaustion, disappointment, heartbreak, and even a little fear all in his EMH field. All he could do in return was gently wrap his little one in his own field thick with love, protection, and care. His voice a little gruff, rough with age but still so gentle for him, in his audial repeating conformations of permission to cry, that he was here, and that he still loved him. Optimus's only response to this was to cry even harder and to sink into the older bot. Grunting with the force Ratchet slowly started moving, pulling Optimus with him so they could be settled on the couch. "I'm not leaving," the bot huffed slightly when Optimus whined in complaint at moving in-between sobs. "Just thought the couch would save me from falling over from standing too long. I'm old remember."
Optimus paused in his sobbing long enough to give a wet giggle before hiding back into Ratchet's neck. He was safe there. Ratchet could feel the little one finally start to breath after about a nanoklik and he pulled a wet cloth from his subspace. "Head up kiddo." He huffed in a soft command. "Gotta get this off before it dries and gets all icky and sticky." He stuck out his tounge to emphasize his point and Optimus giggled again.
Optimus whined a bit when he started wiping his face but soon settled. "Pak." He whined miserably.
"I know I know." Ratchet, although he would forever deny it, cooed softly and kissed his nose. "Do you know what? We can still cuddle and read. Doesn't that sound nice?"
Optimus thought about it for a moment, thumb drifting to his derms absentmindedly, before he slowly nodded. "Duks." He said quietly looking up at him and although it sounded like "dooks" more than the waterfowl the old medic smiled and nodded.
"I'll read to you about ducks kid. No reason we can't do that."
At the confermation his eyes lit up and he settled against his chest expectantly. Ratchet laughed kindly and pet his helm with one hand. "We gotta go get everything ready first kid. Don't you want to still have our picnic inside while I read."
Optimus sat up at that and smiled so brightly at him Ratchet's spark almost broke. Such a simple thing and he was smiling like he hung the cybermoon for him. "Oil!" The baby bot squealed scampering off and bouncing in place.
Ratchet could only laugh again and stand up slowly, joints cracking and poping. "Yeah kiddo- oil. And a pillow picnic. Now run along and bring your sparkling stuff to my hab alright? I'm sure your stuffies miss their favorite bot."
Optimus nodded and bonded off waving excitedly at prowl and exclaiming "oil!" Before leaving the motorcycle bot in the dust.
Ratchet snickered before turning and going to get ready for his little one
🌀 — Optimus regresses involuntarily, causing him to become confused and scared often. Especially if he’s by himself.
﹕When he’s around Ratchet he immediately clings to him, thinking he’s one of his parental figures. ( Either one of his doctors from when he was younger or one of his creative advisors. )
﹕ When with Kup, he tries to act tough and stop himself from crying or whining. Kup understands Optimus though and tells him he doesn’t have to act strong around him, especially not because he’s young.
﹕ It took the others a long time to get used to it when Optimus would randomly start acting like a sparkling. Especially Bee. He tries his best to be an older brother for Optimus when he’s small.
﹕Optimus loves to sleep when little, it distracts him from stress and problems.
﹕He doesn’t regress to a specific age, but he does regress to an age range of a sparkling of about 3 - 5 years old.
﹕Cries a lot, gets scared very easily when in new places of the arc. He stays in one spot of the ship usually, either Ratchet’s office, the main panel room or his office. He doesn’t go anywhere else unless it’s with a cg.
﹕Gets disoriented by his height and strength. Because of this, he sits down a lot, trying to make himself seem shorter. Along with this he doesn’t touch much or play with toys aside from ones that move on their own as to not break them or damage them.

Baby Op is so special to me, many people see him as a cg but I think him being a little works very well too and makes sense.
What do you think would be the cybertronian equivalent to age regression, a genuine question. Idk if someone has already come up with this idea. I think it would be called something like spark regression instead. Idk just had ideas ...

I usually ignore the whole "using the bathroom" thing for Cybertronians. My head conon is anything they eat is absorbed and incorporated into their bodies, making them highly adaptable and efficient at finding fuel sources. Anything they can't absorb will make them throw up or cause their organs to get gunked up, which either has to be burned off as exhaust fumes or have to be manually removed by a medic. Sparklings/Younglings are more sensitive. They can only have highly filtered energon and minerals untill they're older. Their fuel tanks will reject anything else, being too young to filter their own tanks and having organs too small to operate on safely.
Although the topic of your question is not canon to my main fic, I like your scenario and it got me thinking. So here are my thoughts.

Following the events of the story, Knockout would have been there to help Starscream clean his sheets that first night. With professionalism, the medic would not have made a big deal about it; assuming it was a side-effect of the pain blockers or the seeker's injuries. It would not have been the first time he's had to deal with an accident as it comes with being a medic in a war. Sometimes these things happen. It's best to just clean it up and not draw too much attention to it.
Later, when they figure everything out, Knockout and Breakdown would have to find a better resolution. Lacking other options, they would deal with it the human way and wrap an absorbent cloth around Starscream until they could find a better solution. It works well enough and Little Starscream doesn't have an issue with being padded.
Starscream would try to hide it and will it to stop on its own, but the stress would make the problem worse. Triggering him to regress more often and have more accidents.
Adult Starscream on the other hand, would be mortified about having this problem. Never having a childhood and having no experience with sparkling/younglings he wouldn't even know this could happen to someone. He would be embarrassed and frightened about his loss of control. Out of everything that happened while he was small, this would mess him up the most.
Knockout and Breakdown would eventually get Starscream to agree to having timed breaks to empty his wast tank. That way when he does regress unexpectedly, he's less likely to have an accident.
With the help from Shirley and the other vehicons, they would find a solution that would hide the seeker's padding and let Starscream be in public without discomfort or risk of exposure to anyone that shouldn't know.
With the added security of the hidden padding, accidents happen less and less often. It's more common when Starscream's really small. And when it does happen K.O and B.D are there to support and reassur Starscream that everything is okay.
It takes a long time for Starscream to be okay with himself again, his embarrassment and shame weighs heavily on him, but he gets there eventually with the help from his loved ones.

Hightide
I can't believe it! For the fourth time this week he's overworking himself again. I call out to him, using his pre-prime name to keep him little. "Pax, come on. It's time to recharge." He doesn't look at me but I can hear his aggressive typing. I can tell he's getting frustrated that his helm is starting to feel small.
"Come on kid, let's go." I gently grab his arms, pulling him away from the console. I always expect a fuss now but not for him to turn and shove at me to get me away from him. "Go away!" He turns back to the console to keep working but everything is coming out as gibberish on the screen.
"Pax that was rude." I pull him away and turn the console off after saving his work. He tries to protest me but I shut it down quickly. "Listen to me, you do NOT get to shove me because you are upset. Do you hear me?" He nods as I hold both wrists in my servos. "Good. Now, let's go see Prima for a moment, I need to grab a file or two from him."
He shakes his head and starts trying to pull towards the console. "No, you're done for the day." He shakes his helm and tries to get away, only to get highly upset when I pick him up and carry him. I make it an eighth of the way there before I have to put him down because he won stop crying and hitting.
I drag him the rest of the way and we make it to Prima's office. "Prima, your apprentice is overworking again." I let go of him once the door shuts behind us and he takes off to go yell to Prima.
Prima
I kneel down to get more on eye level with my apprentice. "Little, I take it?" I ask Hightide as I reach out for Optimus servo. "Mhm, the kicking, crying and screaming little." I look at Hightide then down at my sweet little apprentice who has tears on his face. "Oh poor sweetspark, what happened?" I pick him up and hold him, completely forgetting about Nexus and Zeta who are stand right infront of me.
He hugs me the best he can as I hold him. "He ruined my work!" He points at Hightide, I can hear his voice starting to slip as well. "Did he now? That wasn't very nice, was it Pax?" He shakes his head and leans against me. "Glitch." He points at Hightide.
I look down at him, he never uses that word! "Nexus- don't you dare laugh!" I look over at him and he's struggling. "Pax, that's not a very nice word Sweetspark." He looks up at me with those all too familiar teary eyes and, well..."mean glitch!"
Zeta
Before I can process what's going on Optimus gets handed to me and Prima drags Nexus out by his foot. "Well, it was nice working with him." I say, mostly to myself. Hopefully Prima doesn't COMPLETELY scrap him. "Hi little spark, how are you?" I sit down at Primas desk with Optimus on my lap.
As per usual he doesn't respond, just looks around for a distraction from his headspace. "Hightide, could you pass me one of his toys please? Preferably the squeaky one?" He nods and tosses it to me and I immediately give it a squeak by his helm. "Pax look, it's squeakers!" I show him the toy and he takes it.
He squeaks the toy and messes with it before ultimately deciding to chew it. "Awww" I bounce him gently, trying to keep him occupied as Hightide packs up his things from today. "How often does he do this?" I ask, glancing at Hightide. "At least 3 times a week."
I didn't expect that...our little Prime is THAT stressed over work? "Poor sweetspark..." I look down at him but by now he just cares about chewing on the toy I gave him. I reach out and gently touch him helm, which in turn gets him to look up at me with big blue optics. "Zeezee!" I smile at the nickname and gently toss him in the air, causing him to giggle.
"Higher!" I toss him again. "Zeta please don't drop him." I turn to see Prima enter the room again, along with Nexus. "I'd never drop a sparkling! Especially him!" Nexus glares at me. "You dropped me AND Micronus when we were younger!"
I shrug. "Yeah but that was on purpose, look at how sweet Orion is, I'd never drop him!" He giggles even though he probably doesn't know what's going on, usually never he does. "I know! You're just a tiny little sparkling, aren't you?" I poke him repeatedly, he just sits there and giggles, happy to be a part of this.
Prima
I smile at the scene, it's adorable! Optimus just sits there and babbles away at Zeta, even though no one really understands. "So, am I taking him now or are you all gonna fuss over him like he's brand new from the allspark?" We all turn to see Hightide standing there, watching us. "I think we should keep him for a while, that way you have some time to study. You have a test tomorrow in class, no?"
He nods and leaves the room. "Okay, now what?" I look to Nexus and quickly respond. "Now, we get to play with the little darling. Although he probably needs to refuel soon... Zeta why don't you pass him to-" Zeta hugs Optimus close. Worth a shot to try.
"Alright, I'll go get him some energon then. Be back in a few minutes." I head to go get some energon for our sweet baby prime.
Nexus
He keeps chewing at the toy, giggling and smiling as we play with him. Zeta hands him to me and I bounce him around, making faces as he reaches up at my face, making tiny babbling noises. When I look over at Zeta he has the play mat set up. "Are we just gonna let him crawl around?" He nods and I lay him down on his back.
Optimus
So many colors! I reach to the things hanging down from the colorful bars over my helm. They're just out of reach. I try to get to them but it's no use, they're too far! "Awww, look at the little spark! Are you hungry?" I look to the familiar voice to see Prima.
Before I can respond he picks me up and pulls me into his arms, holding me so I don't fall- and how dare he! I want to grab all the colors before they go away, but he just smiles and starts to laugh! "Oh I know, I'm just so mean to you, aren't I?"
I yell and smack at him, doing my best to tell him to let go, but what does he do? He shoves a cup of energon into my mouth and tells me I'm "Aww! Who's a good little sparkling?You are!" I protest at him, giving him a glare that would seriously scare him. "Such a cute little thing! Yes you are sweetspark!"
Zeta
I can't help but smile as my oldest brother coos at the youngling, he just looks so happy. "Prima I don't think he's too happy." I tell him, trying not to laugh as the little Prime glares at him. Though it's more of an angry squint than anything.
"But look at him! I'd give anything to know what's going on in his tiny little processor right now." He says while continuing to feed him. "You know what? You do you. At least he's eating. When he's done I'll take him and clean him up before recharge time." He just nods dismissively and goes back to cooing at Optimus.
8 minutes later
I manage to finish one more report before hearing my brother call out to me. "Zeta he's done eating!" I put everything away, stand up and head over to take him. "After this I'll take him back to Hightide, so say goodnight." He pouts a bit but says goodnight to Optimus before I take him to go get ready to recharge.
I carry him down the hallways before setting him in a little basket next to the tub. It's sad though, he's almost too big for it. "Not what that's meant for, but it works. Now, you stay put." He babbles at me and I pretend to understand as I turn the handles to adjust the temperature of the oil. "Really? I had no idea little one." I talk back as he keeps making noises, occasionally I throw in another one of our fellow primes names.
"Oh I know, Liege can be such a glitch!" I poke at him and pick him up to set him in the tub. I set him down in the tub and in true sparkling-prime fashion, he splashes me with the oil. "Seriously?" He pauses, looks at me and does it again. "Do it again and there will be no playtime before you rest." He immediately huffs and makes more noise but I can tell he's thinking about it.
"Pax, no sweetspark." I gently grab his hand before he can do it. He giggles and tries to splash me again. After a moment he stops and we soon finish bathtime. "Okay, now that that nightmare is over, let's get you all dried off. Okay?" He barely even looks at me as I pull him out of the still pretty warm oil.
I dry him off as quickly but gently as I can before carrying him to his shared dorm with Hightide. "Hightide, he's already mostly asleep, do you want me to just lay him down?" He looks up from his studies and nods. "Yeah, if you don't mind." I nod and lay him down on his berth, covering him with blankets.
Hightide
The moment Optimus falls asleep Zeta leaves so I can go back to my studies. I get in another two hours of studying before I notice Optimus stirring in his sleep and I just have to hope he won't wake up. Of course I couldn't get that lucky...
"shhh, go back to sleep Pax, it's okay." I speak quietly as I walk over to his berth. I offer him his pacifier and try to keep him laying down. He fusses and wiggles around a bit but eventually just lays there while I start to tell him a story.
To my surprise it actually seems to work and he goes back into recharge and doesn't wake up even when I accidentally drop something while turning the lights off. I hit the switch and go lay down under my blankets. And just as I fall asleep I hear his tiny voice.
"Hightide?" He speaks sleepily and still mostly little.
"Yeah Pax?" There's a moment of silence.
"Thank you for being here."

To the anon who asked if my head Canon would work with someone like Breakdown, I apologize for the wait and for accidentally losing your ask. (;'-^-)
From what I remember from your question, you doubted my head canon would work with someone who had pupil-less optics like Breakdown or Arachnid; and you are correct. They do not have visible pupils to dilate or go fuzzy as a visual indicator of their regression.
But fear not! After much time to think about it, I have a solution:
Eye brows
Or more specifically, raising the eye brows.
Using the research from the great @macadam and their post here, you can make a character look younger by raising the eyebrows.

Of course I've also rounded his eyes and added the white lights for extra cuteness.

I've also used this approach when I draw regressed Starscream. I think it works well.
The little guy has a couple of toys that look like his Mama (Megatron), which bring Starscream comfort anytime his main caregiver is busy and can't take care of him himself
(Anyways, let me tell you- drawing transformers wearing clothes is NOT as easy as it may seem at first, but it is fun and adorable in the end!)
(Also included Buster, cuz' that's Thundercracker's doggie - it just felt right to draw it too :3 also also! Thundie has plushies that look like himself and his trine mates! I think its really cute)
