I personally headcanon that Mello would have undergone a drastic change had he survived. The whole string of events would transpire an awakening. Suddenly, he would come to crushing terms with all that he had done, and the inkling of self-awareness he chose to turn a blind eye towards would hit him like a slab of rock. I do believe that he would have a period of depression and self loathing. He gives up. There is no ambition left. Because at that point he does not take his inferiority as something he can fix. It is a burden he will carry forever, rather. It is so uncharacteristic of him to just give up like that, but Mello feels stuck. He truly does not know how to move forth. He wants to kick and fight in the manner that was such a seemingly unmovable aspect of himself, but he burnt out long ago. Thus he spends a good while staying in a cheap apartment. Outside is hostile, and so he doesn't move from the confines of the nicotine stained walls. He was not meant for this world. As the ugly emotions brew more, his hatred for Near is deepened and begins to extend to the general world. Until one day all the time he wastes thinking produces something truly fruitful. He begins to take a turn towards healing as the shred of hope that was always hidden in him unveils itself. (The details of this progression and how it comes forth is yet to be decided.) Mello starts taking walks, joins a club, goes to church. The bitterness ebbs. He chooses happiness. And he apologizes to Near. Near forgives him, and as Mello grows closer to the man he once despised with everything he had, he begins to forgive himself.

Ok idk if that makes sense, but I strongly hc Mello during his time at Wammy’s House as a bulimic, like, the guy will, at some point, be too frustrated, angry, and not knowing how to express his feelings and deal with it in a rational way. It’ll keep getting worse until, at some point, he binges (mostly on chocolate), and will compensate by prolonged fasting. It can also be me just projecting, but I just think it can fit him in a way ??
