personal headcanon of mine, which ive seen some people mention too but im not sure if anyone's had this exact interpretation, is that considering lbd is, well, a bone demon, what if her skeleton actually influenced the anatomy of her hosts (well, we don't really know if Ivory Lady was an actual human possessed by lbd the way bai he was, or if that was a genuine shapeshifting disguise, but considering that in lmk she needed an actual host to appear human, i think its safe to assume IL was one too!). if she possessed someone for long enough, the shape of her skeleton begins to merge/take over the shape of the host, altering their body. LBD has been Ivory Lady for years, so she managed to comfortably take shape, meanwhile Bai He has probably been possessed at most for a couple of months? I think there has been some very minor changes (i've seen some people headcanon that she's grown much taller which i like a lot), which might not be noticeable from the outside perspective, but she can very much feel it
