Regulus still had the adrenaline from the eight-hour surgery but could feel the tiredness catching up with his body. Sirius was annoyingly serious and didn't let him finish closing the poor woman before asking him to talk.
"What do you want to talk about, are you okay? Is Remus okay?"
Sirius looked distressed and had something in his eyes that he couldn't catch what it meant. He took off the quirurjical mask and breathed soundly.
"When we brought your husband into the OR, we originally thought we could go in with a bronchoscope and laser, however there was a lot of bleeding, so we had to open him up."
Regulus immediately felt the ghost of a hand grabbing his heart with long claws ready to tear it apart.
"Wait. You...wait a minute. No. Remus said that--" Regulus hated himself for the stuttering but Sirius ignored it.
"When we opened him up, we saw that his tumour had eroded his pulmonary artery," Regulus tried to breathe but found it difficult to let the air in "Dr. McGonagall and I both used extraordinary measures..."
"No, wait...Sirius. No." Regulus could feel the hand tightening the grab making him bleed.
"But his heart could not tolerate the excessive bleeding," Regulus couldn't see his brother's face so he closed his eyes waiting for the next part "he did not survive the surgery. I'm sorry."
When Regulus looked up he saw Sirius' tearing up eyes, irritated and doing anything in his power to not let the tears fell. His ears felt like shutting down so he breathed feeling fire burning his lungs before speaking.
"You're saying..you're saying, he's dead"
Sirius' tears started falling but Regulus could feel his own eyes fighting to not cry. "Yes."
"Say it."
"Reg..." He sounded broken but he couldn't sound like that because Regulus hadn't broken yet.
"When you inform the family, you have to say it so that they know that it's really happening. It's the first thing that you're taught." Regulus could feel he was losing the war between breaking down and staying alive "I never really thought about it...But now I get it, and, um...Sirius..." The first tear fall and it felt like ice freezing everything in its path "I need you...I need you to say it"
"Barty is dead."
The hand tightened so hard his heart felt like exploding. He lost the fight with the tears that started falling and falling and falling and never-ending.
"Thank you." the words hurt in his throat and Sirius made a movement to hug him, he didn't do anything to stop him because he felt like he was falling into a black hole that led straight to hell.
Barty was dead. Barty was dead. He was dead. And maybe Regulus was too.

Barty pretends to be a noble to ask for Regulus hand in marriage and then they pretend to die lost in the ocean, only to go be pirates (Barty was already a pirate)
And then there's this other group of pirates (Captain Potter and his crew) who maintains a really big rivalry with Captain Crouch and his crew. And obviously there will be Black brothers angst and confusion about relationships and maybe death
This is a beginning, but I'm working on it
