When my hearing aid broke, I just couldn't hear anymore. It was only covered by insurance the first time due to the acute onset of my hearing loss being deemed a medical emergency (because all at once I had compromised environmental awareness). A one time health insurance exception.

Hearing aids had to be paid for out of pocket through your audiologist. The law making them more available and thus lowering the cost wouldn't be passed for another decade, so I was looking at needing $2000-$4500 to have it replaced. As a disabled person, I am not allowed to have more than $2000 in my bank account (or in physical assets), ever. If I do I lose both my income and my health insurance. 🙃

Eventually my family managed to crowd fund a replacement on my behalf (I couldn't do this myself due to the limits I must exist within). ASL courses are not considered medically necessary either, btw. I had to go to college to learn ASL and unfortunately outside of deaf spaces, it helps me 0% because so few other people know it.

Insurance actually did cover my eye exam and my glasses, but even then only once per 10 years -- even if my eyesight were to change. Thankfully zenni offers glasses w/ prescription lenses for like $6, so I've never been without a backup pair. I have had to pay $80 out of pocket for an eye exam though to obtain a new prescription when my eyesight worsened after 4 years but I wasn't eligible for another insurance covered eye exam yet. 😑

Anyway, American health insurance is terrible a scam, especially if you require any manner of medical device. Most just straight up are not covered. When I had a root canal, insurance covered the procedure but not the crown. The crown is kind of the most important part. You have to pay for it out of pocket, $900-$3000 per tooth (some companies may offer a 50% discount/rebate if you know how to apply for it).

You could require a pacemaker and wind up paying out of pocket if your insurance provider arbitrarily decides it's not medically necessary. They love automatically denying claims with 0 medical review in the hopes you'll just give up. Always appeal an insurance denial, have your doctor do it if you manage to find a decent one. They take them more seriously than they'll take you, the person whose life is in their hands.
