Fifth Disease?! What happened to Fourth Disease?!
Today Dr. B is going to teach you some old, outdated medicine that will only help you at some sort of medical trivia event. Good times! As you can probably guess, if its called Fifth disease there must be first through fourth diseases, right?! Well there used to be. The list was based on the most common causes of infectious rashes in kids. I’m not sure when the list was in wide use but it was essentially obsolete by the 90s.
FIRST DISEASE - Measles. That’s easy.
SECOND DISEASE - Scarlet Fever. Another easy one.
THIRD DISEASE - Rubella. Seems pretty easy.
FOURTH DISEASE - This is where it goes off the rails. We’ll finish the list and circle back.
FIFTH DISEASE - Parvovirus B19, a.k.a. Erythema Infectiosum
SIXTH DISEASE - Roseola Infantum or Erythema Subitum. Caused by human herpes virus 6B and 7.
So let’s go back. What the hell is Fourth Disease? When the list was in widespread use, Fourth Disease referred to Filatow-Dukes’ Disease. Originally described by Clement Dukes in 1900, there is a debate in modern medicine as to what exactly he was talking about. Some folks think that it refers to Staph Scalded Skin Syndrome, also known as Ritter’s Disease. Other folks, however, believe that Filatow-Dukes’ Disease is actually people misdiagnosing scarlet fever and therefore doesn’t really exist as its own entity. By the late 60s-early 70s folks were already questioning Fourth Disease with a 1979 article in The American Journal of Disease in Children publishing an article linking it to epidermolytic toxin-producing Staphylococcus.
Despite all this, occasional outbreaks of mysterious rashes in children often raise the question, “Is this Filatow-Dukes’?” and occasionally papers show up in journals positing another etiology which would reestablish it as a credible diagnosis.
TL;DR, Fourth disease was a crock of shit so the whole numbered rashes thing fell apart like a house of cards. Fifth Disease just had a nice ring to it so the name stuck.

idk who needs to hear this but nurses who work 16hr shifts aren’t heroes. they’re horrifically exploited workers& they don’t need thanks or applause, they need more colleagues and better labour protection
As a nurse I will say that it’s sad and has been a rude awakening to know that in times of widespread hardship, people will expect you to put your life on the line- and anyone you love and interact with- just to make up for the government and medical field’s greed. We have little staff, either because people are sick or just afraid to come in due to the fear of getting sick. We have little to no protective gear. The ONE face mask they give us per shift doesn’t do CRAP! Administration is no help for us on the actual field and these hospitals are trying to cover up new cases of COVID-19 to hide the fact that their “protection” for staff and patients is inadequate. This week alone, two of my patients have been confirmed cases and 3 medical staff have been confirmed too. It’s not heroic to see your fellow colleagues catch a deadly virus. It’s not fun to see young people my age, OR ANY AGE FOR THAT MATTER, be on a damn ventilator. IT’S TRAUMATIZING! And to think there are people who aren’t taking this seriously still!
In our ED, each nurse is assigned ONE n95 mask for up to FIVE days now.
Admin keeps changing their minds on what the protocols for this are... and, honestly? Those changes seem entirely based on how limited our resources are on any given day.

There’s a tornado warning out (two, technically), so of course I went for a walk outside to see if I could spot it. Never have I seen so many of my neighbors at one time, when there wasn’t a firework show or a picnic.
