You are a US American motorist driving along i-95 during rush hour. You see a pedestrian dart in front of you. You have 2 seconds to make a decision:

Attempt to stop (cause multi-car pile up and kill pedestrian anyway)

Attempt to swerve #1 (cause semi truck next to you to jackknife, cause pileup, kill pedestrian anyway)

Attempt to swerve #2 (into ditch/swamp/retaining wall, 85 mph/137 km per hour, killing yourself, your passengers, and a bunch of other people on the road when your car inevitably sends pieces shooting off into traffic. Also, the pedestrian gets run over by the person behind you, who was NOT expecting a pedestrian and didn't even see them)

Also...the US is usually hotter than Europe...how many miles are y'all planning on walking? Bcs the heat, lack of shade as well as places you can easily stop to rest or buy a drink is gonna be a huge issue. And it's in June!! When the weather can quite easily hit 90F/32C or higher, which will be worsened by walking on hot asphalt surrounded by cars putting out even more heat in direct, unrelenting sunlight while your body also heats up from the exercise as you walk for what surely will be several hours. And that's IF the temp stays at low. We're expecting a massive heat wave this summer with record shattering highs. Wouldn't be surprised if it hits 100F/40C just from the asphalt and cars, and then clears 110F/43C due to the weather.

If you try this you will die, if not from traffic then from the heat. Just pay for the fucking uber. Or, even better, don't fucking come here oh my god?? You're not safe bcs you're white and European ICE has been kidnapping white travelers too. Football can not be more important than your life. Stay home and safe and watch it on TV.

I love the debate in the notes but the people saying "they'll try anyway" are wrong. Walking on a highway is illegal. You are gonna miss the world cup if you try this because you'll be spending it in jail. You won't make it half a mile before the cops show up and stop you, and rn you do not wanna be a tourist from another country blatantly breaking American laws in broad daylight. Nothing good will happen to you.

Also seems like people think walking on the freeway is dangerous because the drivers will try to hit you on purpose. They might, but the real danger is the air pressure. A car driving past you at 80mph sucks you CAR closer to it. And that's a car. You're just a person, you'll get sucked in front of a death machine and die.

But regardless, this isn't going to work because the cops will immediately stop you, so if you wanna actually use the tickets you bought you should take the train or an uber. Otherwise it's a choice between spending it in jail or the morgue.

(Also do you really want an American hospital bill? Really? You really wanna owe a hospital here an astronomical amount of money you will probably never be able to pay back?? Really??)

PSA:

Acetaminophen/paracetamol has a hard stop upper dose limit, above which it becomes extremely toxic.

That limit is 4g (8 “extra strength” (500mg) tablets) in 24 hours (about 2 tablets every 6 hours).

A single dose of 22 extra strength tablets can kill you.

Taking 12 or more tablets per day for more than a week can also kill you (this is about 3 tablets every 6 hours).

Symptoms of overdose take up to 24 hours to manifest, and are fairly difficult to distinguish from other problems. They include abdominal pain (especially right upper quadrant), nausea, malaise, and confusion.

The antidote (n-acetylcystine) must be given within 8hours of ingestion in order to be useful.

After 10 hours the only thing that will work is a liver transplant.

You might think “why would I ever accidentally take so much?”

Well, acetaminophen is in almost everything in the cold/flu/pain aisle. Migraine combos like Excedrin, cold and flu combos like NyQuil, basically anything that says “non-aspirin pain relief”, and anything that’s branded as a fever reducer. It’s all probably acetaminophen/paracetamol.

So the goal of this post is to get you to read the labels on your medications. Because taking taking Tylenol and NyQuil together for a week (like you might if you had the flu) could kill you.

I didn't know this for years, and I took so many pills, sometimes 4 at one go, every four hours, like 16 a day, because of endometriosis and migraines. It took a migraine specialist to explain rebound headaches and overdoses when I was in my 40s. Then I went cold turkey on all OTC drugs to get off the cycle. Please, please, if a couple tylenol aren't working for you, talk to your doctor or find one who will listen to you if you can.

Further to the PSA: Drinking alcohol will lower the dose limit for paracetamol. Don't take paracetamol for a hangover.

Basically what's going on is, your liver has multiple different processes it can use to deal with different substances, but it uses the same one for both alcohol and paracetamol. That process is extremely safe and produces completely non-toxic by-products. There is a strict limit how much of it your liver can do at one time. If it's already at maximum, any more paracetamol coming in will be shunted instead to a different process which produces seriously toxic by-products and will quickly destroy your liver.

By the way, the converse also applies: taking paracetamol will reduce your tolerance for alcohol. I didn't know this in my early 20s and would sometimes take paracetamol for sinus headaches on a Friday afternoon, then go to a pub in the evening and not understand why I was getting drunk so fast on two beers.

the thing that blows my mind is blood transfusions. for literally all of human history up until about 100 years ago if you lost enough blood that was it, you were dead, and then people just figured out how to take blood from other people and successfully give it to you and now you can come in to the hospital with a blood pressure of ohfuck/nope, the same color as the linens and they just pop a tube in your arm and casually give you some stuff that another person donated on their lunch break, and you live long enough for the doctors to find and treat your gastric bleed. Insanely cool.

The fact that we can put fluids into people via IV saves more lives than I can actually communicate. There are so, so many more ways to die when we can't do that. You can go from literally at death's door from an illness you have no other cure for, to Basically Fine, You'll Feel Icky A Bit Longer But You're Otherwise Fine and Your Own Immune System Will Work Now, from sterile saline into a vein.

Or even fucking subcutaneous, under your skin. It still gets into your system faster and bypasses any fuckery going on in your gi-tract.

But you want the other end?

I recently got the answer to a crapload of symptoms of mine and it turned out to be Crohn's. Ileal crohn's.

For most of human history there was literally nothing to do about this but hope and pray that your immune system didn't decide to rip ulcers and lesions in your digestive tract to the point where you bled out, or the point where parts of it died and killed you with sepsis, or enough to build up stricture bands of scar tissue sufficiently to cause impactions or any other really gnarly and unpleasant ways you can die because for some reason your body decides the walls of your digestive tract are the enemy and need to be dismantled cell by cell. (Including a fuckload of cancers caused by the constant damage to the cell wall.)

Even as recently as when most of the younger people reading this were small children, mostly all you could do about it was take corticosteroids when you were in a flare. And that was better than Nothing. But at the same time, corticosteroids have a potential laundry list of side effects and you want to take them as little as possible and for as brief a period as possible. And there wasn't a lot else.

I am on a medication with the proprietary name "Skyrizi" and the generic name risankizumab. It's made from taking antibodies from a non-human source and then modifying their protein sequences to be more similar to human antibodies, after which they modify them further in order to make it so that the literal only thing they do is go into my body and bind to something called "tumour necrosis factor" so that this will stop flagging my own goddamn digestive system walls for destruction by the rest of the immune system.

Please feel free to read that paragraph over again.

Modern medicine isn't perfect; there are many things we're just as helpless against as we were in the Days of Eld, and there are many ways its practitioners fail us. But also we can make a thing that goes into my body and says "hey stop self destructing you MORON!" and I have a much better chance than at any other time of not dying young of bowel cancer or bowl impaction! This is fucking insane.

Vitamins and micronutrients.

There used to be a common, horrific illness that sailors would get, which was mysteriously cured by limes. People know about this one, it's scurvy. But there are other horrible ways to be sick from vitamin deficiency that weren't considered curable at all, and people had no idea what caused them.

Rickets is a disease caused by vitamin D deficiency where your bones get bendy and grow in the wrong shape (it is most apparent in children). It causes permanent deformity and very easy fractures, along with debilitating pain and persistent dental issues. Historically, it was known that milk, and later, cod liver oil, would improve or prevent it, but the reason was not understood until the vitamin was discovered.

Wernicke-Korsakoff syndrome is a complication of alcoholism that leads to psychosis, dementia, and death if left untreated. Severe alcoholics used to just go completely mad before dying, basically. It ultimately results in permanent memory loss (retrograde amnesia), as well as the inability to form new memories (anterograde amnesia). It is caused by the fact that alcohol prevents the absorption of vitamin B1 (thiamine). It is treatable and preventable by giving the patient thiamine shots - if caught early, before permanent brain damage has occurred, it is fully reversible, although the underlying substance abuse issue still needs to be addressed to prevent recurrence.

Pernicious anemia is caused by vitamin B12 deficiency (in turn ultimately caused by an autoimmune issue causing poor absorption). It causes blood cells to be the wrong size and too few in number, resulting in dizziness and fatigue. It also causes neurological symptoms like tingling in the extremities, poor coordination, confusion, and, in late stages, dementia. There was no cure for pernicious anemia in the past. People would simply become anemic and die from it. That's why it's called "pernicious" - that's an old-fashioned way to say "insidious and deadly," named for its slow onset and then-incurable course. Now it is curable with vitamin tablets or periodic injections.

Cretinism, or, less stigmatizingly, congenital hypothyroidism due to iodine deficiency, is a developmental disorder caused by the inability of the thyroid gland to function properly without sufficient iodine. it causes short stature, intellectual disability, infertility, hair loss, and a large lump in the neck known as a goiter (i.e. a hypertrophic thyroid gland). It was historically associated with poor inland populations living far from the ocean (due to the protective effect of consuming seafood, which is naturally high in iodine). We now simply put iodine in table salt, and this disorder is virtually unheard of in regions where this is the case.

Neural tube defects are a leading cause of birth defects, infant mortality, and stillbirth. The most common nonlethal forms of neural tube defects include spina bifida, hydranencephaly, and encephalocele. These defects are caused by a failure of the embryonic structure that becomes the spinal canal to close properly during development, leading the central nervous system to have a distorted shape that may impair cerebrospinal fluid drainage and put pressure on the brain. In severe cases, e.g. anencephaly, the brain/spine essentially develop outside of the body, which is not compatible with life (anencephalic and iniencephalic babies typically die within hours or days; fetuses with more severe forms are usually stillborn if they are not terminated). The risk of these defects is drastically reduced by taking supplemental folic acid (vitamin B9).

Vitamin K is perhaps the most amazing one on this list. Newborns often have very low vitamin K levels due to the fact that it does not cross the placental barrier easily and is not found in high levels in breast milk. It is only produced by gut bacteria, which babies do not have when they are born, and it takes time for them to acquire the right flora from their environment. Deficiency impairs blood clotting, and in infants, can lead to brain bleeds and sudden, unexplained death. Tiny babies would simply die of brain hemorrhaging for no good reason at all. But if they're given a quick shot of vitamin K at birth, that doesn't happen.

We have cured or prevented so many diseases just with vitamins/minerals.

usually this means you're getting most of your calories from either white rice or cassava, neither of which has thiamine.

in the case of rice this wasn't historically a big problem very often, because to get the sort of soft white rice that can lead to it you have to go to the trouble of polishing away the husk that has all the vitamins, which means most of the people relying on it as their dietary staple before industrial-scale processed foods were fairly well off, and ate a varied diet. if it was, you could eat buckwheat about it.

Tips for Writing Injuries

✧ Broken ribs suck. You don’t just “walk it off.” Breathing hurts. Laughing hurts. Existing hurts. Characters with rib injuries won’t be doing heroic sprints.

✧ Concussions aren’t instant naps. Dazed vision, nausea, dizziness, maybe even personality changes, but they’re not going to collapse neatly like in the movies.

✧ Blood loss is sneaky. It’s not just about dramatic pools of blood. It’s dizziness, confusion, and the body getting cold as circulation tanks.

✧ Adrenaline lies. Someone can take a serious injury and not feel it until the fight’s over. That “I didn’t realize I was bleeding until later” trope? Very real.

✧ Twisted ankles are brutal. One bad step and suddenly running is off the table. Even walking hurts like hell. Perfect way to ground a chase scene.

✧ Burns linger. Even small burns hurt more than most people expect. Blisters, infection risk, constant pain, it’s not just a cool scar later.

✧ Dislocated shoulders = useless arm. Characters can’t keep swinging a sword or firing a gun. They’re basically fighting one-armed until it’s fixed.

✧ Shock is a thing. Pale skin, trembling, rapid heartbeat, and eventually disorientation. A character might not even realize how bad their wound is.

✧ Stitches aren’t magic. Getting sewn up is painful and recovery takes time. They’re not instantly battle-ready after a needle and thread.

✧ Scars tell stories. Some fade, some don’t. Some stay sensitive forever. Don’t forget the aftermath when the wound becomes part of the character.

The ground in these places is too compact for water to soak in during wet season which leads to flooding but digging these holes gives the water a place to stop and soak in. And they’re pushing back the desert with this. By just digging holes.

The half-circles are called zaï! They're a traditional farming practice in the Sahel desert, and their introduction + reintroduction can be largely credited to Yacouba Sawadogo, the man linked above! He reintroduced and innovated on the zaï on his own farm in the 1980s, and did extensive outreach (along with scientist Mathieu Ouédraogo) to encourage other farmers to adopt them as well.

He also promoted the use of cordons pierreux, which are basically just lines of rocks to reduce erosion, preserve sediments, and increase water absorption.
