Definitions:

Antizionism: a belief system that is critical of Zionist ideology, WHEN SEPARATE FROM ANTISEMITISM.

This blog draws a very hard line between the two and believes that much of modern ideology being touted as antizionism is actually antisemitism. This blog acknowledges, however, that the two can be separate.

Pro-pal/pro-pally: an intentionally insulting term for the kind of activist that masks their antisemitism, ignorance, intentional disinformation, etc, in a shroud of caring about Palestine. Not to be confused with “Palestine supporter”, “Antizionist”, “people who are critical of Israel” or any other neutral term on my blog. I’m very much in favor of people criticizing Israel in good faith. I am very happy that there are people on the other side of the line from me who care deeply about the people they’re trying to defend. And I would imagine those people also hate Pro-pallies, because any good faith actor in a movement hates the idiots cheapening their cause.

Thoughts on the Israeli Palestinian conflict:

This blog believes that the I/P conflict is a war, and the actions taken by both sides in this war are fairly mundane wartime actions. This blog does not support the use of the term Genocide to describe any of Israel’s actions or goals. This blog DOES support the use of the term War Crime to describe some of Israel’s actions, as well as some of Palestines actions. This blog does not support the enacting of war crimes by any party, ever, under any circumstances.

This blog believes that the territory Palestine has not made good faith efforts to end the war, and that the actions Palestine as a government and military body has taken in the past 10 years are more dangerous to the ideals of progress, human rights, and life on earth, than those of Israel.

This blog acknowledges both Hezbollah and Hamas as current authorities in Palestine, and condemns both party’s actions in the course of this war. This blog defines Hamas as a terrorist group. This blog condemns the attack on October 7, 2023, as an act of terror, and a war crimes, due to the treatment of the hostages taken, and the intentional, goal-oriented attack on civilians and non-residents, as well as their continued torture, sexual assault, and mistreatment of hostages. This is one of myriad war crimes both authorities have committed.

This blog believes that the Israeli military and defense body has made many missteps in dealing with the engagements, and committed war crimes. This blog does not excuse the following actions confirmed by multiple sources:

1. The destruction of food sources slated for civilian use

2. Several large scale bombing of civilian areas. This blog recognizes the fact that Palestinian leaders are using their civilians as shields which is ALSO a war crime, but this blog does not excuse the bombings of these locations even if they are oriented towards a goal of military success and not one of genocide. The end result is the same and “we told them to leave” does not wash when a government is in full awareness that these civilians are under threat by their government.

This blog does NOT see the pager attack in Set 2024 as a terrorist attack or war crime, and instead, given that the goal of assassination of military and government targets was the goal, and the overwhelming outcome, this blog completes this attack as a fair play war tactic.

This blog does not treat every civilian casualty as a war crime. Best efforts must be made to reduce civilian casualty, and in this case, they were. In the case of the carpet bombings they were not. This blog feels for all civilian casualties in this war, including the two children killed in this attack.

This blog believes the Palestinian people are overwhelmingly victims of their own government as well as the war in general.

This blog is in favor of a two state solution that acknowledges Palestine as its own territory, as well as Israel. This blog is NOT supportive of any form of one-state solution in either direction, including a merging of the peoples and cultures of these two territories.

Every cut is a story someone told me, or a time I saw them cry. Every cut is someone else, opening the wall of my ventricles and  atria with a razor blade they don’t know they’re wielding. 

Every time I try to know a person, they cut me open. I have as many cuts as I have memories and they’re constantly bleeding, some are oozing or seeping, some are gushing but none of them are healing.

Why?

Sometimes men see the cuts. Sometimes they see the blood in my tears when it runs together with all my other bodily fluid. They want to help, they want to heal the cuts but they’re like Edward scissorhands, incapable of touching without wounding. They always leave more cuts than when they started.

There are vampires. I don’t need to talk about them. You know what vampires are.

I’m low on blood. I’m low on the pumping, bright red force that powers my body. Sometimes people see the flush in my cheeks, they see the blood, brought to the surface and they think “isn’t she vibrant? Isn’t she bright! There’s so much of her!”

But it’s just the fascination that comes with the sight of blood. 

I’m so tired. I just want to rest.

One thing that worries me about the use of AI is whether or not it can worsen people's dementia and alzheimer's in the future. When my grandmother was first diagnosed, we got her math activity books. Now, my grandmother never had a formal education, but we did our best to keep her sharp, get her to do math and writing activity books, sudokus, playing board games that required some level of strategizing with her. Her family is prone to alzheimer's and dementia (both her siblings had it and deteriorated very very very quickly, which yeah, scares the shit out of me being her granddaughter) but she was the one whose mind lasted the longest, she only passed away two years ago, at 88, ten whole years after her initial diagnosis and sure, she had forgotten things, recipes and where she put her glasses and appointments, but she never forgot any of us, ten whole years in, she still remembered us. Now, this may have been luck, but doctors always said the constant mental work + companionship + medicine helped her a lot. So I'm thinking, these people who are now relying on AI for everything, from email-writing to thinking what's for dinner to casual conversations, I've even seen people rely on it to calculate what time they should leave their house if they need to be at a place at a specific time and their commute lasts X number of minutes. As if that's not... the simplest math operation possible? You shouldn't even need a calculator for that!!! Idk I don't know how long it'll take us to see the effects of this + exposure to brain-rotting short form content that is completely meaningless + people addicted to right-wing conspiracy style media. Idk I'm very worried. Please, read, read complicated books! Take up a book on philosophy and try to decipher it and make your own opinions on it, please buy a maths activity book and relearn how to do math, please get a hobby that involves lots of thinking and concentrating. PLEASE!!!

As a neurologist, I’ll give you the pretty name for it: cognitive reserve.

The way I explain it to my patients is that our neurons don’t regenerate. They make connections with each other and that’s it. If you don’t use your brain, they make fewer connections and, if one of them dies, you’re gonna miss it, because that was the only one that knew how to do X. Now, if each one of them has many, many connections, you won’t notice the difference when one of them dies. The others pick up the slack.

As of 2024, 45% of dementia risk factors are modifiable. Relevant to this conversation, 5% for less education and 5% for social isolation.

We absolutely are going to see the reflection of this, but it’s gonna take decades and it’ll be too late. So, for the love of your brain, pretend that it’s a muscle and make it work. People complain about “when am I ever gonna use this maths formula in my life?” You’re not. You’re teaching your brain to think logically. Those sinapses will be there for when you need to figure out your week’s schedule. English classes taught me how to interpret data and how to convey it in this text so it’s clear and you understand what I’m saying, not because I needed to justify why the curtain is blue.

Make your brain know how to do different things. Logic games, puzzles, taking care of a garden even if small, planning a church’s event or birthday, learn a new instrument, learn a few words in another language, look at a calendar every day, do some manual labor if possible. Do not, I repeat, do not let your brain get rid of sinapses by letting AI do everything. Your brain uses 20% of your body’s energy — do you really think it’s going to maintain connexions that aren’t in use?

Most cases of Alzheimer’s are sporadic, meaning no family history. Family history of a first-degree relative with Alzheimer’s starting before they were 80yo increases your risk in 2-3x on average.

TLDR: Yes. From the knowledge we have today, AI will increase the number and severity of dementia cases.

if your trauma is that severe that you can’t look at a human body without being triggered, you need to stay fully off the the internet until you’re deep in treatment. That’s not a “get out of my space” that’s a “there is no space space on public property, we cannot assure your safety”

I want to address a lot of discourse, jokes, and takes I’ve seen about the fact that American Ambulances and ER visits are expensive as fuck, that boil down to “fuck no I’m not taking an ambulance, I don’t have $3000”

So one. Yeah it’s completely fucked up. Beyond unreasonable. The worst parts of American capitalism and health care failure combined.

But

That is not a reason to not call an ambulance when it is needed.

So to put that into action, below is a non-exhaustive but pretty exhaustive set of lists and ideas on when that line ticks over.

Situations where it is appropriate to weight the worth of an ambulance ride against the need for one, or to allow another person to do so. These are ALL still situations where you should go to the ER or seek medical intervention. This is just about whether you should drive there or call the ambulance:

There is bleeding of less than a pint from one major wound, but you have fully stopped it successfully and they seem lucid

There is an avulsion of a single phalange or ear, and you have stopped the bleeding and the person is calm and conscious

There is a single broken bone that you can easily stabilize such as an ankle or a rib

The person has had a known complication of a known disorder and has tools to handle it(such as an asthma attack where the person has their emergency inhaler) and they have stated that they do not want paramedic intervention.(exceptions to follow in second list)

A panic attack or other psychosomatic episode where the person is clearly dealing with emotional distress causing physical symptoms, and they have asked that you not call an ambulance(exceptions to follow)

Any other injury or condition that looks bad to you but wherein the person dealing with it seems lucid and capable of transporting themselves to a hospital or getting non EMS transport

With that, a list of situations wherein you should always call an ambulance. Note: in these situations it IS okay to ignore the victims wishes. I know that sounds counterintuitive to the principles of body autonomy by which our generations try to live, but there’s a switch where body autonomy is less important than making sure someone has a fucking body to autonomize:

You can’t stop the bleeding. In any situation.

Any avulsion of more than a phalange or ear(I.e. a hand or two fingers)

A potentially broken bone you cannot stabilize, such as a femur or pelvis or several broken bones

Any cardiac symptom(where you were already thinking of calling an ambulance)

The person has hit their head (again, bad enough you were thinking of calling an ambulance already of course) or completely loses consciousness. If someone has this happen then denies EMS, guess what, they are no longer capable of making that decision because concussions fuck with your judgement. You cannot trust someone who has potentially had a TBI. Ditto if someone who has not ever had a seizure has one. A seizure in the absence of an epilepsy or diabetes diagnosis is a SERIOUS symptom and needs immediate medical attention. See below for those two diseases and their sort of special rules.

We talked above about epileptic seizures. This one is TRICKY. There’s a fine line for epileptics where it switches over to “ignore their wishes, get an ambulance” and each epileptic is going to be different. The best thing you can do with a person with a known seizure disorder, is to find someone who knows about their case. If you see someone have a seizure at an event, once they are through the seizure, try to find someone they know well who can tell you what their average seizure lengths are, what forms of behavior are normal or abnormal after the seizure (many people act strangely or erratically after epileptic seizures, some people get aggressive, some groggy, some loopy, it’s a toss up), and follow their lead. If you cannot find someone they know, use your best judgement to judge whether they seem lucid and go from there.

The exceptions to the above are of course, if they hit their head during the seizure, follow rule 2, and if they have a seizure or quick succession series of seizures lasting more than 5 minutes, call an ambulance. And do this even if their legal guardian is screaming at you not to. More than 5 minutes in a tonic-clonic(convulsive) seizure is a serious medical emergency and anyone who treats it like it isn’t shouldn’t actually be a caretaker. Let someone else sort that part out, but get them help.

Anaphylactic reaction, EVEN IF TOU HAVE ADMINISTERED EPINEPHRINE. You won’t have people with anaphylactic issues fighting you on this one too often, they already know this, but an EpiPen is a stopgap solution, if you are using an EpiPen on someone they absolutely still need to go to the hospital immediately, and need paramedics there in case the reaction outlasts the epinephrine, which they often do. If someone fights you on this, ignore them. Always always always go to the hospital

A drug OD, EVEN IF YOU HAVE NARCAN. AGAIN. Masan is a stopgap. It works shorter than the drugs. Additionally, I put this one on here with a PSA of: in almost every US municipality, it is illegal to take legal action against someone in possession of drugs if you find the drugs in the course of responding to an OD. Even deep red states like GA have this in place. And if it wasn’t, then it’s still better for you to do life in prison for possession than to let another person die. But you don’t have to worry about that because you are almost certainly legally protected.

A psychotic episode. This is the exception to “persons brain is making their body do unpleasant medical things”. With every other example of this, there are typically skills you can use to help with the psychological element, or it will self resolve, or they will last a car ride. The exception is a psychotic episode. If someone is having a true psychotic episode, they need professional intervention, and, unfortunately, you should not trust that you can safely get them there. A psychotic episode is characterized by derealization, and a person who does not know they are real is not someone you want near you in a car without restraint. Have a professional help with that. Do not attempt to transport a person in a severe psychotic episode, as they could hurt you or themselves very easily without meaning to.

A vehicle accident. This isn’t so much “call an ambulance for every accident” but more “if you called 911 for the accident already, have everyone involved talk to EMS.” This is actually good for insurance reasons, because if you make a medical complaint at the time of an accident, it can help with establishing causation later. If you call 911 for a car accident, have everyone talk to them.

This is a VERY non-exhaustive list, but it’s a list of situations, several of which I’ve lived through myself, where I have seen people online getting the wrong impression about how to handle this. My advice is, if you’re in doubt about yes or no one an ambulance, call the ambulance. $1200 of medical debt is ALWAYS worth the peace of mind. I say this as someone looking down at a $700 bill just for paramedic response to my last asthma attack where I forgot my inhaler. It’s gonna suck paying that off, and I don’t regret it at all, because by the time they got there, all doubt of if I needed them had vanished.

Debt is a lifelong issue but that matters less if you live a long life.

A good breakdown. The *vast* majority of emergencies may require a visit to the ED but not an ambulance ride- however dialing 911 may still be a smart idea in very rural areas where transport time is considerably longer.

I would add that childbirth is also a perfectly reasonable reason to get an ambulance on the way, especially if no caregivers experienced with delivery are present and something appears to be going wrong for either mom or the baby.

I'd like to add a precautionary line: if you call emergency services for a psychotic episode, please please please ask them not to send cops if the person is not behaving violently and if they absolutely must, demand a Crisis Intervention Team and NOT ye-ole police officer. If you say someone is having a psychotic episode and don't specify this, they may send cops without telling you, and cops are NOT trained well enough to handle psychosis episodes. They can and have escalated things and put mentally ill people more at risk to themselves and others than the episode itself.

Your parents ended up emigrating to Israeli in 1988 when you were in university. You wore a yellow pin honoring the October 7 Israeli hostages at The White Lotus premiere in February. I always wear it if I’m on a red carpet and a press line.

Public sentiment on Gaza seems to have shifted a lot since then. I wonder where you are right now on the issue? Where I am is either a full magazine or no comment about it, because two or three sentences in a profile are not enough to deal with the issues. I wear the hostage pin because there are innocent people who were taken from their homes. Most of them are peace activists who lived in border communities where they were ferrying sick kids to hospitals and working with people from Gaza constantly. There are Holocaust survivors, there are children who were taken, there are people being starved and tortured and raped who have no access to the Red Cross. People are rightfully talking and thinking about all the civilians that are in danger everywhere else. But those people in tunnels, it’s now 600 days they’ve been there, they’ve been forgotten entirely. And so I wore the pin once and the hostages’ families got in touch with me and they thanked me enormously. I now am aware that they are watching me and that it matters to them. If my son or sister or daughter or father was being kept in a tunnel somewhere and weighed 25 kilos now, or may have been strangled or shot, and it felt important to me that some actors somewhere wore the yellow hostage pin, then who am I to not wear it?

So when it comes to more nuanced arguments about Netanyahu and the right-wing lunatics in the cabinet, or whether the IDF is or isn’t doing things, or this new Gaza Humanitarian Foundation is or isn’t handing out food correctly, or whether Hamas’s press releases should be printed as fact, and whether there aren’t journalists in there — there are so many complicated arguments. Unrelated to being smart but one time I was on acid and my friends and I found the game Takenoko. I decided that my goal was to feed the cute panda. That is one of three interlocking goals in the game, all of which serve one another, but also can be cut short by one another, so typically you have to balance all three against one another at once. But I wasn;t interested in winning, I was on acid and there was a panda. He had to be fed.
