Late into the good ending, D goes on a much stronger dose of antipsychotics for a time. They make him dramatically more "present" and able to hold emotionally intense or difficult conversations without becoming overwhelmed and retreating. He eventually goes off of it, as he's much happier when he's more psychotic, and this stronger dose of medication interacts poorly with his pain/joint meds, leaving him dealing with daily pain and unable to exercise (which he otherwise does daily, because he genuinely enjoys it). He keeps the stronger dose around to switch onto if he feels the need to do so, but it's always temporary.
This is the sort of outfit D wears while he's on the stronger meds. His fashion sense is still on the cute side, but much more muted.
And, of course, he has his little Jean-Paul bag. :)
