Dean Winchester with a lip piercing, I'm talking hoop snake bites or a singular spider bite (lower lip, side. So not necessarily spider bite anymore) so when you kiss him, you feel the cold metal against your lips... I could even see him with a labret, but it would again be a singular silver hoop; not gaudy, un assuming, something eye-catching that would drive the ladies (and men) wild.
He'd get it cause it draws people's eyes to his lips and feeds into his "sex appeal." (At least that's how he'd explain it to Sammy.)
I can also see him getting nipple piercings, but that is unrelated to my point on facial piercings.

He could get an eyebrow piercing, too, but I think it would look wrong with the lip piercing, so it would have to be one or the other. And I just know John would hate it regardless, so it would be like an act of rebellion after his death, and Dean has moved on a bit from the grooming.
Sam has double lobe piercings (I like to think all Winchester's have a set of Dimond earnings they get as like a: "You made it to twenty while being a hunter congrats!" type thing), so Sam gets his second lobe piercing with Dean, but his first is with Jess, and he wears these little dangle cross earrings.
Like I can imagine Jess piercing his ears and putting the little silver studs in, and then when he goes jewelry shopping with her one day he sees these silver crosses and he's just drawn to them so she encourages him to get them and that's all the push he needs.
Usually, I'd insist on a cross necklaces, but Sam isn't a necklace guy- Dean, on the other hand, tottaly has a cross necklace (maybe even a gift from Sam to replace the samulet(rip) when he gains faith in later seasons a way to represent Cas, as a peace offering, or just a bonding thing so they have more thing's that match; their boots, their Dimond earrings, Sam's cross earrings and Dean's necklace, etc).
Dean and Sam wanted to take Adam to get his ears peirced but he "dies" at age 19 so he never gets them done and then he's trapped in the cage with Michael so the Winchester family tradition never quite happens.
Of course, (delusionally,) when Adam gets out of hell with his husband in toe and meets back up with Sam and Dean to put down the boundary that he and Michael just want to be left alone (to enjoy married life together in bliss) Dean and Sam still want to try and be Adam's older brothers so Adam let's them despite it all and on his "20th" birthday they take him to get his ears peirced.
Though I think Adam would already have his ears peirced (I can even see Kate helping him do it), so it would be a Sam situation where he's got double lobe peircing on-top of his right ear cartilage peircing.

I think it would be really funny if it's in the "gay" ear, like it was unintentional, and cass didn't know, but it would be funny as hell.
Though I like the idea of Angel's being in a sort of Kori'ander situation where their vessles skin is impenetrable (obviously not but yk) so they just can't get their ears peirced I like @dutchrt 's idea for this a lot.
