well you see. the Q continiuum as a whole is built upon the notion that the Q are superior entities, and Q (singular)'s conception of his own identity is similarly tied to him as a higher being. the first time he appears, his goal is to literally judge the inadequacy of humanity. so i do think that his ensuing fascination with and then attachment to Picard threatens the core conceit of, well, himself and his entire race. the idea that Q might consider Picard a friend, a person to which he can meaningfully connect, and therefore an individual equal to himself in some way threatens Q's inherant perceptions of who he is and how he fits into the universe. it essentially proves Picard's own belief that there isn't such a thing as a superior being, merely a more powerful one. and it forces Q to grapple with the fact that he has caused harm to living creatures who are in fact capable of as much true interiority and personhood as he is.
so in conclusion, yes, while he can be glib about it, Q would never truly and sincerely admit to being Picard's friend as easily as he would admit wanting to fuck him. thanks for coming to my ted talk
