Informed consent is not informed: a revised version of the form I copied straight from Planned Parenthood.
In this post I want to expose the lie of informed consent. Luck you, I saved my informed consent forms from the day I got prescribed testosterone at Planned Parenthood in April 2018. I was prescribed testosterone after my first appointment without even having gotten a blood test. I assumed I was informed, since I was fed on a steady diet of youtube transition videos/forums for years.
But I was wrong. In fact, the informed consent sheet makes efforts to prevent full understanding. Having spent the better part of this year deep-diving into the far reaching consequences of using cross-sex hormones, I'm gonna attempt to create a more informed consent sheet. In red are my notes.
What is masculinizing hormone therapy?
Masculinizing hormone therapy is a medicine that can be used as part of your gender transition process. the medicine is called testosterone. quick question, what is PP's definition of medicine?
what are the benefits? there are no real medical benefits that'll increase the life expectancy of the patient or anything. the benefits are purely psychological based on medically detrimental physical changes.
your voice will become deeper. Your voice will become deeper because the cartilage of your vocal cords are thickening. When testosterone thickens adult female vocal chords, their proportionately smaller lungs often cannot push enough air to create a typical male voice. It has been observed that some ftms are permanently hoarse or their voices sound grainy or nasally. it may even be difficult to speak at all. As we have established, the vocal cartilage will thicken. But actually, all your cartilage thickens. all of it. It is not local, it is universal. universal means brains, arteries, and heart valves. This is why trans men are at greater risk of heart attacks.

you may grow a beard and mustache.
hair on your body will be thicker and grow faster.
Your clitoris with get bigger. This is called clitoromegaly and can have two severe complications. When your clitoris gets big enough to permanently rest outside the clitoral hood, touching it can become painful. Even when it's just rubbing on your underwear. Even when you're just trying to walk. This hypersensitivity may cause problems with sexual intimacy and naturally , may i add, exacerbate dysphoria. The 2nd severe complication is less common, and occurs when the clitoral tissue grows faster than the clitoral nerve endings. This can dampen or totally eradicate sexual arousal. here is a post compiling some experiences from FTM reddit users.
your muscles may get bigger and stronger. except your bones are not large or hard enough to support such increased weight, which can lead to universal muscle and joint pain.
you will probably stop having a period. this is because testosterone induces menopause and attacks the reproductive system.
you will notice less fat on your buttocks, hips, and thighs and more on your belly.
increase your red blood cell count the increased red blood cell count means your blood will be thicker, which means increased risk of blood clots. remember the cartilaginous heart valves are already potentially thicker, their openings narrower. so it may take a smaller clot to increase heart attack
increase the amount of fat and/or cholesterol in the blood so, increased risk of clots
increase the chance of getting diabetes
there may be long-term risks that we don't know about.
What are the side effects?
Acne Acne is a sign of a hormone imbalance. If you are using hormones not meant for your body you are inducing a hormone imbalance. there is no way to be an otherwise healthy female on testosterone and not be hormonally imbalanced. Two trans men in Gender Dysphoria Alliance (go see on youtube), who have been on testosterone for over 10 years each, share a diagnosis of aggressive psoriasis, another skin issue concerning the overgrowth of skin cells that also has a hormonal cause.

mood changes testosterone is a known mood stabilizer. A lot of trans guys say "they even out" on testosterone. This is not because testosterone is adequately addressing dysphoria or your mental health problems. It's because testosterone boosts your mood no matter what. using testosterone to treat your mental problem means many of the fundamental issues that cause a person's mood disturbance or dysphoria will not be addressed. testosterone is a placebo. speaking from personal experience, too, I noticed I had a much smaller range of emotions, was more angry, and weirdly, no creative drive to speak of. That might be a personal thing.
increased sex drive Again, you might think the new sexual confidence you have is because your dysphoria is being addressed but believe it or not, you are just, for the first time in your life, more horny than you are self-hating. You are not actually becoming well adjusted. You're on drugs. You're on steroids.
vaginal dryness. remember how your periods stop because testosterone is attacking your reproductive system? Let me elaborate. Estrogen stimulates the regrowth of vaginal cells but you're wiping the estrogen out. your cells are not replacing themselves as fast as they're dying. So your uterine and vaginal walls will become thinner and thinner. It will become easier to tear. the more it tears and heals itself, the more scar tissue. if this happens enough the nerves that tell you when you have to pee can burn and scar over and you could become incontinent. Additionally, the healthy vagina is (say it with me!) A self-cleaning oven, because the vaginal PH is very, very acidic and the acid kills bacteria. But testosterone lowers the acidity of the vagina, that is, makes it more basic. this means the FTM vagina is both prone to tearing and to infections. please read my comprehensive post here breaking down a study on this. If an infection makes it into the vaginal tear, you could end up with sepsis. At the very least, penetration will become extremely painful. There are also many documented cases of trans men experiencing intense pain after every orgasm.
When the uterus is atrophied enough, it may collapse or prolapse. this can lead to pelvic organ prolapse.
You may need a hysterectomy to stop the agonizing cramps/cancer risk, which can also happen. the loss of your uterus can lead to pelvic organ and rectal prolapse, and urinary and fecal incontinence, plus an increased risk of developing dementia later in life.
There are local estrogen creams you can apply in the affected area, but they can only work to the extent that the estrogen overrides the huge amount testosterone you're taking. it can reverse some of the damage, kind of like duct tape on the Hoover Dam. If you've got burned nerve endings by then, well, that's irreversible.
I'd also like to add here that none of these "side effects" are side effects. they are simply direct effects of testosterone that are unmarketable. they are not optional. Every single female person using testosterone will experience vaginal atrophy, for example. it is impossible to separate the use of testosterone from vaginal atrophy.
Besides testosterone, what are my other choices?
you could do nothing. Another way to masculinize is to have surgery. If you are interested in other options, talk to your doctor or nurse why isn't planned parenthood mentioning talk therapy, which is proven to help dysphoria? why are they framing it as "you could alter your body permanently or do nothing and suffer"?
In conclusion, the information Planned Parenthood gives is not enough for a person to truly give consent. Though all of the effects mentioned are accurate, PP does not give an explanation for why those effects occur, and as a result gloss over the severe health problems that implicitly arise as a result of testosterone that can and will impact thousands of womens' healths. I'm shocked (but not) that this is legal.
