Paralysis + Wheelchair User: After being shot in the spine, Johnny was paralyzed from the waist down. Before the start of the Steel Ball Run, he uses a wheelchair, but once the race starts, he's usually on his horse, Slow Dancer.
Gregor:
Prosthetic Arm: As a result of experiments done on Gregor and several others, his right arm has been replaced with an insectoid arm. It renders him unable to use his arm outside of combat most of the time. In several of his Mirror Identities, his arm is shown to be a proper prosthetic instead of just an insectoid arm.
PTSD: As he was a soldier in the Smoke War, alongside the experients done on his arm, he has PTSD from all of that trauma.

Vision Impairment: While it is uncertain whether or not the Contract Seal actually blocks his vision, he uses an eyepatch most of the time to hide it. He had to learn to navigate seeing through one eye.
Scootaloo:
Undeveloped Limb: Scootaloo's wings are underdeveloped, meaning she will never be able to properly fly like her pegasus peers. However, she can use her wings to propel herself forward on her scooter.

Wheelchair User: Nessarose was born without use of her legs due to her mom chewing milkweed in an attempt to prevent her child from turning out green like Elphaba had been. She uses a wheelchair because of this.

Lost a leg
I did more research, and Peeta lost his leg due to an infection caused by blood poisoning, gangrene, and a deep cut by Cato. The Capitol deemed his leg unable to be saved, and thus amputated it and replaced it with a prosthetic.
Izumi:
Missing Organs: After attempting to revive her stillborn child with Human Transmutation, she lost some of her internal organs in the process, leaving her prone to fits of coughing up blood and rendering her physically unable to become pregnant again.

Phos:
Memory Loss: In this series, losing parts of your body means that you lose the memories that were in those limbs. Phos lost both arms, both legs, and even their head, as well as one of their eyes. They do not have a lot of memories left by the end of the series.
Multiple Amputee / Multiple Limb Replacements: As mentioned above, Phos has lost all of their limbs, as well as their head, and one of their eyes. All of these have been replaced by something else.
PTSD: While their memories are scattered these days, Phos has PTSD from all of the trauma they have suffered over the years; between losing multiple close friends (including Ghost Quartz and Antarcticite) to the Lunarians, and their inability to help due to their weak constitution.
Weak Constitution: Due to their Hardness Level being 3.5, one of the weakest, Phos is unable to wield a sword and fight like the rest of the Gems. In fact, even their teacher, Kongo, yelling causes Phos to shatter. While things change as their limbs get replaced, this is how they start.
Hallucinations: Phos has been shown hallucinating Antarcticite, reliving the moment where they died in front of them over and over.
Beldaruit:
Chronic Health Issues: Beldaruit usually stays in bed unless he has to leave it, due to his chronic health issues that leave him bedbound.
Wheelchair User: Beldaruit uses a magical equivalent of a wheelchair to get around, as he cannot walk otherwise.
Hypermobility: Beldaruit has hypermobile joints and brittle bones. His Wiki page states that his mobility aids are indicative of him possibly having Ehlers-Danlos Syndrome (EDS).

Respiratory/Stomach Injuries: After defeating All For One for the first time in the past, he suffered critical injuries that affected both his stomach and respiratory system. This created a time limit on how long he could stay transformed, preventing him from being the Symbol of Peace for as long as he was able to before the incident.
Yang:
Amputee: During her fight with Adam Taurus in Volume 4, Yang lost her right arm while attempting to protect Blake Belladonna, her teammate and future girlfriend. Thanks to Pietro Polendina, Penny's father and creator, she was able to get a robotic prosthetic arm to replace it.
PTSD: For a long time after the above incident, Yang developed PTSD due to the trauma it all caused. She would get flashbacks, nightmares, and would even hallucinate Adam.

Age Regression: Not necessarily a disability, but important to note, as he likely uses age regression in order to cope with the various traumas he has suffered from since his creation at the hands of Heperu.

Social Anxiety/Stage Fright: Nene's social anxiety and stage fright has been shown to disable her, to the point where, in the main WonderlandsxShowtime story, Nene had to use Nene-Robo, a robot Rui created for her, in order to communicate with others and even perform on stage. While she's slowly getting better at it, this has been indeed shown to disable her in the past.

While I personally headcanon it to be fibromyalgia or something akin to it, the game has provided no actual name for the illness in question. She has previously been hospitalized because of it. She has to be careful not to push herself, as she gets fevers if she overdoes it. However, she pushes herself regardless so she doesn't worry her friends.

Missing Limb: Black Raisin Cookie, for unspecified reasons, does not have one of her arms.
